If it's an emergency, call 911. For a past event, FMCSA's National Consumer Complaint Database at nccdb.fmcsa.dot.gov accepts complaints from people who witnessed a safety problem on the road. You can also call 1-888-DOT-SAFT (368-7238), weekdays 8am–8pm ET.

What information should I include?

Date, time, location and direction, the company name or USDOT number, plate or unit numbers if you saw them, and a factual description. Leave out anything you didn't see.

Where do I find the USDOT number?

It's usually on the truck's cab door. You can look up a carrier on FMCSA's SAFER Company Snapshot.
